HomeMy WebLinkAbout1972_VILLAGE OF LIVINGSTONIMPORTANT INSTRUCTIONS —ALL CLERKS (Read these instructions before completing these preprinted Statements of Taxes and Indebtedness) (Also read the instructions contained in Assessment and Tax Rolls) GENERAL Enclosed please find three partially preprinted Statements of Taxes and Indebtedness. Use the large pre -addressed manila envelope found in the taxroll to mail the white copy back to the Department of Revenue after you have completed all three copies. The Statement is due on or before the third Monday in December. Be sure this is the Statement for your tax district. If you are no longer the Clerk, please give this to the new Clerk. The Statement of Taxes and Indebtedness bound in your Assessment Roll (PA-532) and Tax Roll (PA-657) will remain in the rolls. Should you have already completed them you will find that the information will transfer to these forms easily. Much of Page 1 has already been preprinted for you: (1) Name of Tax District and whether it is a Town, Village or City, (2) Name of the County. (3) County and Tax District Codes. (4) Year of this Statement of Taxes. (5) School District Code Numbers (for use by Department of Revenue, Department of Public Instruction and Wisconsin Board of Vocational Education).' (6) Legal names of School - Districts (Common, Union, Vocational). (7) Your Name and Address. — The Department of Revenue is attempting, by preprinting, to use stored data from .last year's records. With the help of the Department of Public Instruction and Wisconsin Vocational Board, last year's records have been updated, and printed on Page 1 for you. This has been done to aid you in properly reporting the legal names of all your school districts, and the data for them, in the proper section of Page 1. Be sure you list the assessed value, levy and any State Trust Fund Loan opposite the corresponding school district name. These names may be preprinted in a different order than shown by you in your roll copy of Statement of Taxes. The Statements of Taxes and Indebtedness filed with the Department of Revenue and with the County Clerk should contain the same information appearing in the "Statements of Taxes and Indebtedness" annexed to the assessment and tax rolls and the statement retained in your files. Check each school district name. Check all three types. You should have received an apportionment from each district shown. Ina few municipalities of the state, school district reorganizations and municipal annexations may have taken place which became effective between the time this form was preprinted and the time which you send it to the Department of Revenue. If your municipality fails into this exceptional case, the following procedure should be followed: A. Should there actually be fewer school districts than are shown, line out the improper preprinted district(s). B. Should there actually be more school districts than are shown, enter the legal name of the additional district(s) on an available line. Do not attempt to code such additional district(s). C. Should a school district legal name be incorrect, line it out and enter the correct legal name immediately above in the space provided on the same line. NOTE: Do not tear Page 1 apart from Page 2. IMPORTANT: Carbon paper has been left in for your convenience - Be sure to remove these carbons before making any entries on Pages 3 and 4. If carbons are not removed, your entries on Pages 3 and 4 will cover up your entries on Page 1 and 2. Should erasures be necessary, be sure you protect the second and third copies, as well as make the correction on them. Should you prefer to work without the carbons, remove them by using these instructions. (1) Open the set to full length so that Page 1 is at top and facing you. (2) Grasp left stub (about 5/8" wide) at top of Page 1 with left hand and grasp extreme right margin (about '/zwide) with right hand at top right corner. (3) Pull outward, tearing along perforation of left margin. (4) As the perforation tears, move your hands down the form pulling carbons out left side of set. Please complete the certification on the white and yellow copies. Be sure your statement is complete and accurate before affixing your signature to the certification. Tax credit moneys for your district will not be released until all supporting documents (Statement of Assessments - PA-528, Tax Credit Certification - PC-201, Statement of Taxes and Indebtedness - PA-665, white copy) are on file, complete, correct and reconciled.
