PA-658 416*Af g_.yc,(REV. 2/74) IMPORTANT INSTRUCTIONS - ALL CLERKS (Read these instructions before completing the Statement of Taxes and Indebtedness bound in your Assessment and Tax Rol Is) GENERAL The Statement of Taxes and Indebtedness bound in your Assessment Roll (PA-532) and Tax Roll (PA-657) will remain in the rolls. The Statement of Taxes and Indebtedness (PA-665) to befiled with the Department of Revenue and with the County Clerk will be mailed to you by the Department of Revenue and when completed must contain the same information that appears onthe "Statement of Taxes and Indebtedness" bound in your Tax Roll. Complete the form In the roll, then copy the required information on to the pre-printed forms mai led to you. Use the largemanila envelope (addressed to the Department of Revenue and found in the tax rol I) to mai I the white copy back to the Department of Revenue. The Statement of Taxes and Indebtedness isdue back to the Department of Revenue, Bureau of Local Fiscal Information and Analysis, on or before the third Monday of December. The report must be made with strict accuracy because the taxes reported are used to determine general property tax rel ief and shared taxes for each municipality. Also, the reports areused for the purpose of administering the laws for the taxation of rai Iroads and other publ ic util ities and special taxes. Clerks are therefore requested to exercise great care to make thereports correct in every respect. Make all entries as indicated. Do not group together in a single total items which are capable of separate statement in different lines and columns of this report. The figures, in detail, are needed for various statistical purposes and the value of the report would be greatly ·Impaired if the details were not shown. There are a few instances In which a city or village is located In more than one county. In such cases there must be a report to the County Clerk of each county in which the city orvillage is located. In such reports each of the various types of taxes raised In the entire munic, pality should be apportioned between the parts of the municipality located in differentcounties in accordance with the assessed valuation,of the property in each county, so that the report to each County Clerk wi I I show only the amount of taxes raised in that part of the cityor village lying in his or her county. A like apportionment should be made in stating the indebtedness of such city or village. INSTRUCTIONS The tax rol I should be completed first; then the rol I copy of the Statement of Taxes and Indebtedness should be fi I led out; and then the copies should be mai led to the Department ofRevenue and the County Clerk. The Statement of Taxes and Indebtedness has two important purposes. They are: (1 ) to summarize all taxes and charges placed on the tax roll for col lection;and (2) to enable you to uniformly report for your municipality such taxes as wei I as indebtedness and newly incurred special assessments to the Department of Revenue.Take a few moments and study the form before completing It. . Notice that each page is divided into Sections separated by heavy lines. Each Section is SPECIFICALLY INTENDED. for aparticular tax, special assessment or special charge. Each Section is identified by a letter of the alphabet. Each portion of a Section is identified by a line number, with the exception of,the twelve I ines for school districts. These lines will be identified by a school district code number on the partia!ly pre-printed copy of the Statement of Taxes and Indebtedness mailed toyou. Each column is headed by a number. These numbers are identical to the column numbers on the rn, 11 rate worksheet (PA-523). Never make entries in the shaded areas.Do not change any inquiry by striking put words, or by writing in others. If you are not absolutely certain how to enter a given tax, special assessment, or charge, please explain itsnature very carefully under the "Remarks' section (page 3) so that the Department of Revenue may reclassify your entry if it has not been, properly classified.Sections A, B, C, D and E should include general property taxes only. Be very careful not to enter special assessments and charges, which should be reported under Section F.General Property Taxes are taxes levied by a uniform mi.ll rate upon the taxable general property of an entire town, village or city, or by a uniform mill rate upon all of the property locatedin some portion therein, such as a school, fire, lighting, utility or sanitary district, etc., which covers only part of the municipality.NOTE: DO NOT reduce Section E by the amount of State Tax Credits (per Sections 79.10 and 79.12, Wisconsin Statutes). The taxes reported in Section E should Include all generalproperty taxes prior to reduction by State Tax Credits. PAGE ONE - INSTRUCTIONS SECTION A (Page 1 ). The State tax is apportioned to your municipality each year by your County Clerk on the County Clerk's Apportionment Sheet. Show this on line A-1, Column 11.SECTION B (Page 1 ). County taxes are also apportioned by your County Clerk. The County Clerk receives from the State the items he must apportion to your municipality under 8-2 on hisapportionment sheet. These are the items you must show on I ine B-2 of the Statement of Taxes and Indebtedness, Column 11. You must show on I ine B-3a, Column 11, the portion of countytaxes which apply to your entire municipality. On lines B-3b, c and d you must show any part of the county tax which applies to only part of your municipal ity. I f you are not sure ask yourCounty Clerk which of the county tax items, if any, apply to only part of your municipality. DO NOT MOVE ITEMS FROM SECTION B ON THE COUNTY CLERK'S APPORTIONMENT AHEET TOSECTION C OF YOUR STATEMENT OF TAXES AND INDEBTEDNESS. SECTION C (Page 1 ). Lines 4a through 6d are intended to be used for special purpose districts for which a tax is levied using total assessed value of the special purpose district tocompute a mill rate which is then applied to the assessed value of property In such special purpose districts. Such taxes should not be shown in Section F as Special Assessments. TheMill Rate Worksheet (PA-523) is designed to handle special purpose districts as part of Section C. Lines C-7 through C-9 contain items certified to your municipality by your County Clerk. These items are municipal taxes, not county taxes. DO NOT MOVE ITEMS FROM SECTION C ONTHE COUNTY CLERK'S APPORTIONMENT SHEET TO SECTION B OF YOUR STATEMENT OF TAXES AND INDEBTEDNESS. Lines C-10 through C-13 are municipal items which have not been financed by other sources of revenue, therefore necessari ly become the general Droperty tax levy for municipal purposesin order to make up the difference between budgeted expenditures and estimated revenues. On line C-10 enter the taxes for local highway purposes.C-12. This applies only to cities in which public schools are administered by a Board of Education as distinguished from a school district, Such cities should enter opposite C-12 theamounts raised as a general city tax for the payment of principal and Interest on school indebtedness, the purchase of school sites and .construction of school buildings, etc. Amountsincluded in the Board of Education budget and raised as a local school tax should appear under Section D. Lines C-14 and C-15 will be discussed later under Column 13. You wi Il have either overrun or underrun, but not both, after you have completed the extension of general property taxes on the ro 11. Line C-16 is the total of Columns 11, 12 and 13 with the Column 13 entry being total municipal taxes actual ly levied.COLUMN 2 (Page 1 ). Space is provided for you to enter the names of sanitary sewer, metropol itan sewer, and fire protection districts (on I ines 4a through 5c), and to identify such other special purpose districts, such as a lighting district, which may be located in your municipality Con lines 6a through 6d). On lines 3b through 3d you are able to Identify those portions of the county tax (B-3 on the apportionment sheet) which apply only to a part of your municipality. An example might be aspecial (handicapped) education tax. Because one school district in your municipality may provide such a service and the second school district doesn't, the county tax for special educa. tion is to be levied only over the area of your town contained in the second school district and it is a county tax, not a school tax. COLUMN 5 (Page 1 ). Enter the assessed value of property in any special district identified by you on lines 3b through 6d. Such special purpose districts wi Il contain less assessed value than the municipality as a whole or if they contain all of the value, their taxes will be part of the municipal budget. COLUMN 11 (Page 1 ). Enter the taxes apportioned and certified to you by your County Clerk. or certified to you by clerks of a Sanitary District Board or Metropolitan Sewerage District Commission, or other special purpose districts, or as determined necessary from the municipal budget document ( i.e. if expenditures exceed revenues in that budget, the difference becomes the municipal tax levy). COLUMN 12 (Page 1). Enter amounts of surplus funds ("cash") used to reduce taxes listed in Column 11. CAUTION: Surplus funds ("cash") Should only show on other lines after linesC-10, C-11, C-12 and C-13 have been first reduced to zero, or In the event there were no municipal taxes to be levied. Further, if surplus funds ("cash") are used to reduce taxes such money must be used to reduce taxes which apply to the entire municipal ity on ALL taxable real estate and ALL taxable personal property. In other words, do not reduce a tax which appl ies to only part of the property in your municipality or to only certain classes of property. School taxes should only be reduced as a last resort. COLUMN 13 (Page 1 ). This column contains the amounts of taxes actually levied and extended on the tax roll. OVERRUN - UNDERRUN - Lines C-14 and C-15 are prokided to enable you to show the exact amount of total taxes on your Statement of Taxes and Indebtedness that was actually leviedon the roll. It is suggested that al I overrun (or underrun) for state, county, municipal and school taxes be shown as one figure rather than divided among the various taxes. If you use combined mill rates you wi I I not know wh ich tax caused the overrun Cor underrun). COLUMN 14 (Page 1). You may enter the various mill rates. Notice some lines are shaded because any mi Il rate entered would only be an average mi Il rate and actual ly would not apply to any portion of the municipality. PAGE TWO - INSTRUCTIONS SECTION D (Page 2). This section provides a summary of school district taxes. SECTION E (Page 2). This section provides a summary of all general property taxes actually levied on your tax roll. SECTION F (Page 2). Enter special assessments indicating in the spaces provided to the left of Column 13 the amount of each type of assessment belonging to the town, village or city and the amount entered in the tax roll for individuals holding contractors' certificates and special assessment bonds. Do not confuse special assessments with the frequently used phrase "special taxes." For instance, in the municipality it is often said that a special tax was levied in order to bui Ida particular bridge or to purchase a road machine. Such taxes are GENERAL taxes and should be entered in the proper subdivisions of Section C. Special assessments are not based on property values but are taxes imposed on only some, not al 1, parcels of real estate within a jurisdiction, normal ly to defray the cost of capital improvements to those parcels such as pavements, sidewalks, sewers, drains, etc. These improvements are presumed to benef it the publ ic and are of special benef it tosuch parcels of property. Special charges are for some special service (such as cleaning snow off sidewalks, or cutting weeds) performed for the owners of property. These charges may be thought of as service fees collected on the tax roll. SECTION G (Page 2). No entry should appear unless the local water or sewer plant is owned and operated by the town, village or city reporting, and then the amounts entered thereafter should represent the amount of delinquent rentals which consumers have failed to pay and which are carried Into the tax roll for collection. Tax levies to pay for hydrant rental or any water service to be paid by the municipality either to a private or municipal plant should be entered in inquiry C-13. Amounts to be entered for sewers are only del inquent sewer service charges authorized by Section 66.076 (4) (7). SECTION G (1 ) (Page 2). All omitted property taxes from previous year(s) should be recorded here. SECTION H (Page 2). Opposite -Woodland Tax Lands" enter the woodland tax land acres and the woodland tax. Only acreages are shown in assessment and tax rolls, These acre- ages are taxed at 20 cents per acre. If you multiply 20 cents per acre times the acreage, the result should equal the tax roll amount you have entered in Column 13. SECTION I (Page 2). Opposite "Forest Crop Taxes," enter only the forest crop tax. Private Forest Crop Lands * entered under regular classi f ication are taxed at 10 cents per acre, or 20 cents per acre if such acreage is under Department of Natural Resources order of entry dated after December 31.1971. Special Private Forest Crop Lands are taxed at 20 cents per acre, Show acres and taxes for each class and enter the total taxes for al I classes in Column 13. * Private Forest Crop Lands entered by private owners must be assessed by municipal assessors, but these values are used only in case the lands are later withdrawn. Do not enter contributions received from the State. in the form of state aids under the heading -Forest Crop Taxes." SECTION J (Page 2), Occupational Taxes - Enter the appl icable number of mink farms, bee colonies, tons or bushels. These can be obtained from the assessment roll. Enter the taxes in Column 13. These can be obtained from the tax roll. The number multiplied by the tax per unit should equal the entry in Column 13. SECTION K (Page 2). Please show total, this should be the total taxes on the tax roll. SECTION L (Page 2). The special assessment total from Page 3 should be posted here. DO NOT PLACE THE TOTAL ASSESSMENT OF THE GENERAL PROPERTY OF THE MUNICIPALITY IN THIS SPACE. COLUMN 2 (Page 2) Section D. Enter the names of elementary district(s) (if any) and district(s) operating high schools on the first set of seven I ines, the Union High School District(s) (i f any) on the next set of two I ines and vocational district(s) on the last set of three lines. IT IS SUGGESTED THAT YOU ENTER SCHOOL DISTRICTS IN THE SAME ORDER AS THEY WERE PRE- PRINTED BY THE DEPARTMENT OF REVENUE ON THE PRIOR YEAR'S STATEMENT OF TAXES AND INDEBTEDNESS, OR ON THIS YEAR'S STATEMENT OF ASSESSMENT. This should aid you in reporting school taxes to the Department for the proper school district. In the past, school district data was reported to the Department erroneously because clerks have arranged school districts in a different order on their rol I copy of the Statement than they are pre-printed on the copy Of the Statement of Taxes and Indebtedness mailed to you in December. Data was then copied erroneously. COLUMN 8 (Page 2) Section D. Enter School tax certified to you by each school district clerk. COLUMN 9 (Page 2) Section D. Enter amounts of school district state trust fund loans (if any) certified to you by school district clerks. This item should not be combined with school tax. It should be stated separately in Column 9. COLUMN 10 (Page 2) Section D. Enter handicapped tuition (if any) in the school district(s) to which it applies. Handicapped tuition is certified by your County Clerk, usually on his ' annual apportionment sheet. This item should not be combined with school tax. It should be stated separately in Column 10. COLUMN 12 (Page 2) Section D. The practice of reducing certified school taxes is discouraged (see earlier convnent under Column 12, Page 1) but if surplus funds ("cash") are used toreduce school taxes enter such amounts of surplus funds oppo'site the respective school districts affected. COLUMN 13 (Page 2) Section D, Enter school taxes actually levied on the tax roll. BE SURE TO ENTER TOTALS OF COLUMNS 8, 9, 10,12 and 13. COLUMN 14 (Page 2) Section D. Enter the mill rates for each of the school districts in your municipality. PAGE THREE - INSTRUCTIONS Indebtedness: Complete this schedule if it applies to your municipality. New Special Assessments: Enter here the total of al I new principle obligations created by special assessments made, assessed or levied for the year against specific abutting parcels or pieces of real estate irrespective of any pre-payments or instal Iment provisions and irrespective of whether or not they are included in the tax roll, together with the purposes for which same were levied. Do not include instal Iments due this year on obl tgat ions for special assessments first made, assessed or levied in any prior year (see Section 68.01 of the Statutes). Please enter total of new special assessments on the last I ine of Page 2. This sum is important in the computation of tax credits and state shared taxes (under Chapter 79) for your municipality. Remarks Section: Clarify any of your entries which you believe require an explanation. Changes in Assessment: Should the total assessed value shown in your tax roll disagree with that of your assessment roll (line 26 of Statement of Assessment), please show in the schedule provided here the lines, numbers and the changes to be made on your Statement of Assessment. If you report a change for I ines 1 through 26 and I ine 30, you must also report a change for the school district which is affected. The assessment roll and tax roll must agree. Tax credits for your district will not be released until all supporting documents (Statement of Assessment (PA-528), Tax Credit Certification (PC-201), Statement of Taxes and Indebtedness (PA-665), white copy) are on file, are complete, correct and reconci led.
