(1) The treasurer of each town, city, or village shall, unless exempted under subsection (2), execute and deliver to the county treasurer a bond, with sureties, to be approved, in Ease of a town treasurer, by the chairman of the town, and in case of a city or village treasurer, by the county treasurer, conditioned for the faithful performance of the duties of his office and that he will account for and pay over according to law all state and county taxes which shall come into his hands. If such bond is executed, or the condition thereof guaranteed by personal sureties, the amount of the bond shall be double the amount of state and county taxes appor- tioned to the town,. village or city, provided that the amount of such bond shall not exceed the sum of $500,000. When such bond is executed, or the condition thereof guaranteed, solely by a surety company as provided in section 204.07, such bond shall be in a sum equal to the amount of such state and county taxes, provided that the amount of such bond shall not exceed the sum of $250,000. The county treasurer shall give to said town, city or village treasurer a receipt for said bond, and file and safely keep said bond in his office. (2) The treasurer of any municipality shall not be required to give such bond if the governing body thereof shall at any regular meeting by resolution obligate such munici- pality to pay, in case the treasurer thereof shall fail so to do, all state and cbunty taxes required by law to be paid by such treasurer to the county treasurer. Such governing body is authorized to so obligate such municipality. If the governing body of the municipality has adopted a resolution as specified in this subsection, it may demand from its treasurer, in addition to the official bond required of all municipal treasurers, a fidelity or surety bond in an amount and upon such terms as may be determined by the governing body. Such bond shall run to the town or village board or the city council, as the case may be, and shall be delivered to the clerk of the municipality. A certified copy of such resolution filed with the county treasurer shall be accepted by him in lieu of the bond required by subsection (1). The official bond executed pursuant to section 19.01, required of municipal treasurers, shall extend to and include the liability incurred by any town, city or village whose governing board shall adopt and certify to the county treasurer a resolution in accordance with this subsection. The Statement of Taxes and Indebtedness (PA-665) to be filed with the Department of Revenue and with the County Clerk will be mailed to you by the Department of Revenue and when completed must contain the same Inforn·ation that appears on the -Statement of Taxes and Indebtedness" bound in your Tax Roll. Complete the form in the roll, then copy the required information on to the pre-printed forms mailed to you. Use the large manila envelope (addressed to the Department of Revenue and found in the tax roll) to mall the white copy back to the Department of Revenue. The Statement of Taxes and Indebtedness is due back to the Department of Revenue, Bureau of Local Fiscal Information and Analysis, on or before the third Monday of December. The report must be made with strict accuracy because the taxes reported are used to determine general property tax rel ief and personal property tax rel ief for each municipatily. Also, the reports are used for the purpose of administering the laws for the taxation of railroads and other publ ic util ities and special taxes. Clerks are therefore requested to exercise great care to make the reports correct i n every respect. Make all entries as indicated. Do not group together in a single total items which are capable of separate statement in different lines and columns of this report. The figures, in detail, are needed for various statistical purposes and the value of the report would be greatly impaired if the details were not shown. There are a few instances in which a city or vi I lage is located in more than one county. In such cases there must be a report to the County Clerk of each county in which the city or village is located. In such reports each of the various types of taxes raised in the entire municipal ity should be apportioned between the parts of the municipality located in different counties in accordance with the assessed valuation of the property in each county, so that the report to each County Clerk will show only the amount of taxes raised in that part of the city or village lying in his or her county. A like apportionment should be made in stating the Indebtedness of such city or village. INSTRUCTIONS The tax rol I should be completed first; then the rol I copy of the Statement of Taxes and Indebtedness should be fi I led out; and then the copies should be mailed to the Departrnent of Revenue and the County Clerk. The Statement of Taxes and Indebtedness has two important purposes. They are: (1) to summarize all taxes and charges placed on the tax rol I for col lection; and (2) to enable you to uniformly report for your municipal ity such taxes as wei I as indebtedness and newly incurred special assessments to the Department of Revenue. Take a few moments and study the form before completing it. Notice that each page is divided into Sections separated by heavy I ines. Each Section is SPECIFICALLY INTENDED for a particular tax, special assessment or special charge. Each Section is identified by a letter of the alphabet. Each portion of a Section is identified by a line nurnber, with the exception of the twelve lines for school districts. These I ines will be identified by a school district code number on the partial ly pre-printed copy of the Statement of Taxes and Indebtedness mailed to you. Each column is headed by a number. These numbers are identical to the column numbers on the mi I I rate worksheet (PA-523). Never make entries In the shaded areas. Do not change any inquiry by striking out words, or by writing in others. If you are not absolutely certain how to enter a given tax, special assessment, or charge, please explain its nature very carefully under the "Remarks" section (page 3) so that the Department of Revenue may reclassify your entry if it has not been properly classified.Sections A, B. C, D and E should include general property taxes only. Be very careful not to enter special assessments and charges, which should be reported under Section F.
