Pursuant to Sec. 19.84, Wisconsin Statutes, notice is hereby given to the public that a meeting of the
Unified Community Services Board will be held on Wednesday, October 21, 2015 at 7:00 p.m., meeting
at Unified Community Services, 200 W. Alona Lane, Lancaster, Wisconsin
Agenda
1. Call to order/roll call.
2. Certification of compliance with the open meetings law.
3. Consideration of Consent Calendar - The following items may be approved on a single motion and
vote due to their routine nature or previous discussion. Please indicate to the Chairman if you would
prefer separate discussion and action.
A. Adoption of the agenda.
B. Approval of the minutes of the September 16, 2015 Board meeting.
4. Payment of the bills for September, 2015.
5. Reports
A. Financial Report for September, 2015 – Zack Armstrong.
B. Personnel report for September, 2015 - Cheryl Knapp.
C. Chairman's report – Mike Tiber.
D. Program Director's report - Kathy Rink.
E. Reports from other Board members.
Following: A resolution to receive the reports and place them on file would be in order.
6. Introduction and opportunity for members of the audience to address the Board. Chairman will
allocate time.
7. Agreement for 2015 Audit.
8. Resolution regarding Transfer of Community Options Program Funding from Grant County
Department of Social Services to Unified Community Services.
9. Adjournment-Next Meeting, November 18, 2015.
