Pursuant to Sec. 19.84, Wisconsin Statutes, notice is hereby given to the public that a meeting of the
Unified Community Services Board will be held on Wednesday, December 20, 2017 at 7:00 p.m., meeting
at Unified Community Services, 200 W. Alona Lane, Lancaster, Wisconsin
Agenda
1. Call to order/roll call.
2. Certification of compliance with the open meetings law.
3. Consideration of Consent Calendar - The following items may be approved on a single motion and
vote due to their routine nature or previous discussion. Please indicate to the Chairman if you would
prefer separate discussion and action.
A. Adoption of the agenda.
B. Approval of the minutes of the November 15, 2017 Board meeting.
4. Payment of the bills for November/December, 2017.
5. Reports
A. Financial Report for November, 2017 – Zack Armstrong.
B. Children’s Community Options Purchase – Zack Armstrong.
C. Personnel Report for November, 2017 - Cheryl Knapp.
D. Chairman's report – Mark Stead.
E. Agency Director's report – Jeff Lockhart.
F. Reports from other Board members.
Following: A resolution to receive the reports and place them on file would be in order.
6. Introduction and opportunity for members of the audience to address the Board. Chairman will
allocate time.
7. Adjournment-Next Meeting, January 17, 2018.
